Serenissime ac [Invicti]ssime Rex et Domine, domine cl[ementissim]e ....

[Hu]millimam perpetuae meae servitutis commendationem.

Scri[psit] ... in servitiis Serenissimae Maiestatis Vestrae per annos aliquot venerabilis d[ominus] ... Baltasar a Lublin sacerdos et artium magister, qui ad praesens est reverendissimi domini Culmensis cancellarius et in iis terris Serenissimae Maiestatis Vestrae non inutilis servitor. Pro eo humillime apud Serenissimam Maiestatem Vestram intercedo, cum Varschoviae prius diu et honeste vixerit, ut illi exspectativam ad canonicatum Varschoviensem ex singulari sua in me gratia clementer concedere velit. Habitura illum Serenissima Maiestas Vestra ad Dominum Deum oratorem unamecum perpetuum.

Qui ut eandem Serenissimam Maiestatem Vestram quam diutissime prosperet sospitetque in omnibus, quantum possum, precor impensius meque Serenissimae Maiestati Vestrae supplicissime commendo.
